The Smoke Free Arizona Web site provides information regarding the “Smoke-Free Arizona Act” (A.R.S. § 36-601.01 and § 42-3251.02). The Act restricts smoking in public places and workplaces including but not limited to: bars, restaurants, workplaces, stadiums, reception areas, theatres, and designated non-smoking hotel rooms. The site provides compliance information (including downloadable signs for posting in the workplace), county and local links and resources for smoking cessation, and options for reporting smoking violations including an electronic violation reporting form.

Information Collected and Stored Automatically

Log Files

ADHS (through its service provider) collects limited information about your visit and stores it in a log file as you browse through the Web site. A log file consists of data regarding visits to a Web site. The following information will be collected:

• The Internet domain name from which you access our site (for example, aol.com, if you have an America Online account);
• The date and time of your visit
• The type of browser and operating system of your computer (for example, “Windows IE” browser and “Window XP” operating system).
• The pages you visit
• The address of the web site you came from when you came to visit
• Your IP address (an IP address is a number that is automatically assigned to your computer when you surf the Web)

We use this information to measure the number of visitors to the different pages of our site and to help us make our site more useful to visitors. Our service provider will not collect additional information or use the log file information except as stated in this policy. Log file information will not be disclosed to anyone outside of ADHS except to help identify individuals engaging in unauthorized attempts to access or damage the contents of the Web site or as otherwise required by law.

This web site does not use “cookies” (small bits of data that a web server stores on your hard drive) to obtain information about you or track your visits to our site.

Information You Provide

The Smoke Free Arizona Web site contains online forms and links to send us emails in which you may provide personal and/or demographic information or reveal your email address. If you choose to provide us with personal information – by filling out and submitting a form or sending us an email – we will use that information to respond to your message or request. Smoking Violation Reporting

Providing personal information when making a smoking violation complaint is optional per A.R.S. § 36-601.01(G)(3), “A person shall not be required to disclose the person’s identity when reporting a violation.”

Information received by us when you report a violation will be used to investigate the complaint and may be shared with others as required by law. If you do not wish your name or other personal information to be shared, please file an anonymous complaint (do not provide personal information).

Alternatives to Providing Personal Information on the Web

The Smoke Free Arizona Web site offers services that you might otherwise transact in person. Our goal is to provide you with a more convenient and efficient means in which to interact with the Arizona Department of Health Services. If at any time, you choose not to proceed with a web transaction (e.g., filling out a form or sending us an email), you may obtain the same services by contacting the agency by phone or in person.

Access to Information Collected Via the Web

ADHS uses a third-party service provider to collect data from forms filled out on our Web site. This data is made available only to ADHS, and our service provider employs security methods and measures to ensure the integrity and confidentiality of your data.
